Application forms

1. Send the completed application form (the original plus 2 copies) to the Office of the Director at TDR. Completed forms may be submitted electronically (to tdrgrant@who.int), however, the Investigator must follow-up by sending either the original form (with original signatures), or the two pages which require original signatures by mail or courier:

• the responsible Administrative Authority (institutional endorsement)

• the Chief Financial Officer and Principal Investigator (budget page)

2. Send a copy of your proposal to the WHO Representative in your country

3. Keep a copy for your records

1. Send the completed financial report form by post or courier to:

Electronic submissions of financial report forms (via email) and submissions by fax will NOT be accepted as original signatures are required.
